Lost or Stolen Equipment

If your equipment has been lost or stolen, please contact us as soon as possible at (800) 403-1010.

What happens next:

1. We'll verify your account and the equipment on file

2. We'll check with your insurance to determine coverage for a replacement

3. If covered, we'll coordinate getting you new equipment

4. If not covered, we'll explain your options including private pay

What you may need:

β€’ A police report (for stolen equipment)

β€’ A new prescription from your doctor (required by most insurance plans)

β€’ Updated medical documentation if your original order was over a year ago

Please note: Insurance coverage for replacement equipment varies by plan. Medicare and most insurers require a new prescription and may have waiting periods before replacement is covered.
